Q) Do you follow the Latest Dental Fee Guide?
A) Yes, Chinook Smiles follows the latest Alberta Dental Association Fee Guide.
Q) Are you open evenings & weekends?
A) Yes, Chinook Smiles is now open on Tuesday evenings till 7 pm for your convenience. We are also open alternating Saturdays from 9 am to 4 pm.
Q) Do you direct bill?
A) Yes, Chinook Smiles is able to direct bill your insurance. Please inquire with our Administration team regarding your specific insurance requirements.
Q) I am fearful of visiting the dentist! What options do you have for me?
A) Dr. Lisa Mallett is pleased to offer anxious or fearful patients Sedation Dentistry. Dr. Mallett is licensed to perform Oral Sedation, Nitrous Oxide Sedation, and IV Sedation therapy.
Q) What are your hours of operation?
A) Monday, Wednesday and Thursday from 7:00 am to 4:00 pm, Tuesday from 7:00 am to 7:00 pm, Friday from 7:00 am to 12:00 pm and alternating Saturday from 9:00 am to 4:00 pm. We are closed on Sundays and statutory holidays.
Q) Do you accept insurance?
A) Our clinic accepts all insurance plans. We will assist in submitting claims from the office as most cases are done electronically. Please contact our office with any questions about pre-determination, and we would be pleased to answer any questions you have.
Q) Do you direct bill?
A) Yes can direct bill your insurance (also called assignment)
Q) What types of payment do you accept?
A) We accept cash, checks, and all major credit cards.
Q) How do I cancel or reschedule an appointment?
A) If you need to cancel or reschedule an appointment, call the office at (403) 252-1404. Please keep in mind that all appointments must be cancelled 48 hours in advance. A cancellation fee of $50.00 may be incurred for inadequate notice.
Q) Why do I have to pay a cancellation fee?
A) You will only be required to pay a cancellation fee if you do not give reception adequate notification of your cancelled appointment. Our patients’ health is important to us! Sudden cancellations create large gaps in the schedule where other patients in need of treatment could have been treated more efficiently. As a courtesy to other patients, please give advanced notification if you are unable to keep your appointment.
